Caramel sauce not dissolving into milk

Hey o/
I made some caramel sauce for my coffe, recipe is:
250g sugar
200g cream
70g butter
1/2tsp salt
It came out amazing, it is kinda liquidy at room temp and spreadable solid in fridge as expected. but the thing is when i stir this into cold milk, then add my cold brew. 80% dissolves and remaining 20% turns into rain drop like particles. I prefer my drinks cold so heating milk with caramel is not an option. thanks for tips in advance.
