What happened to my miso in my miso soup?

I’ve made miso soup multiple times without running into this issue (shown above). The miso clumped and clouded, and the soup barely tasted like miso.

Made dashi with Kombu + mushrooms

Didn’t let anything boil, added miso right before I intended to serve.
